Injury Guidelines For Members
1.Injury must be reported to the officer or person in charge of the scene or event immediately.
2.The officer or person in charge will then determine if you need immediate medical attention and the extent of the attention that is needed for the type of injury.
3. If you are sent for medical treatment you must notify our department insurance liaison and then the McAdoo Borough Secretary who will then contact Workman's Compensation
4.You must then file a form with our Accident and Health Policy. This form can be downloaded from Provident's website
5. After this form is completed, it must be given to the fire company insurance liaison for submission.
6. All related doctor appointments and treatments must be communicated to the liaison at all times.
7. At the present time, the insurance liaison is Robert Leshko, 570-233-5987.
8. Failure to adhere to these policies may result in workman's compensation being compromised or denied.
